Indication:An oral prescription medicine used to treat certain types of tumors. It belongs to a class of drugs known as hypoxia-inducible factor-2 alpha (HIF-2α) inhibitors. This innovative therapy works by targeting a specific protein pathway that plays a crucial role in the growth and development of certain cancers, offering a new approach to treatment.

Welireg (Belzutifan) is a hypoxia-inducible factor inhibitor indicated:

  • Von Hippel-Lindau (VHL) disease  for treatment of adult patients with von Hippel-Lindau (VHL) disease who require therapy for associated renal cell carcinoma (RCC), central nervous system (CNS) hemangioblastomas, or pancreatic neuroendocrine tumors (pNET), not requiring immediate surgery.
  • Advanced Renal Cell Carcinoma (RCC)  for treatment of adult patients with advanced renal cell carcinoma (RCC) following a programmed death receptor-1 (PD-1) or programmed death-ligand 1 (PD-L1) inhibitor and a vascular endothelial growth factor tyrosine kinase inhibitor (VEGF-TKI).

  1. Anemia: Welireg can cause severe anemia that can require blood transfusion. Monitor for anemia before initiation of, and periodically throughout, treatment with Welireg. Transfuse patients as clinically indicated. For patients with hemoglobin <8g/dL, withhold Welireg until ≥8g/dL, then resume at the same or reduced dose or permanently discontinue Welireg, depending on the severity of anemia
  2. Hypoxia: Welireg can cause severe hypoxia that may require discontinuation, supplemental oxygen, or hospitalization [see Dosage and Administration (2.2)]. Monitor oxygen saturation before initiation of, and periodically throughout, treatment with Welireg. For decreased oxygen saturation with exercise (e.g., pulse oximeter <88% or PaO2 ≤55 mm Hg), consider withholding Welireg until pulse oximetry with exercise is greater than 88%
  3. Embryo-Fetal Toxicity: Based on findings in animals, Welireg can cause fetal harm when administered to a pregnant woman. In an animal reproduction study, oral administration of belzutifan to pregnant rats during the period of organogenesis caused embryo-fetal lethality, reduced fetal body weight, and fetal skeletal malformations at maternal exposures ≥0.2 times the human exposures (AUC) at the recommended dose of 120 mg daily.

FAQ

Welireg is the brand name for the medication Belzutifan, an oral prescription medicine used to treat certain types of tumors. It belongs to a class of drugs known as hypoxia-inducible factor-2 alpha (HIF-2α) inhibitors. This innovative therapy works by targeting a specific protein pathway that plays a crucial role in the growth and development of certain cancers, offering a new approach to treatment.

Welireg is specifically approved by the FDA for the treatment of adults with Von Hippel-Lindau (VHL) disease who require therapy for associated renal cell carcinoma (kidney cancer), central nervous system (CNS) hemangioblastomas (tumors of the brain or spinal cord), or pancreatic neuroendocrine tumors. It’s not a general cancer drug but is highly targeted for these specific tumor types linked to VHL disease.

Belzutifan works by inhibiting the activity of HIF-2α, a protein that, when overactive (as often seen in VHL-associated tumors), promotes tumor cell growth, survival, and the formation of new blood vessels that feed the tumor. By blocking HIF-2α, Welireg helps to disrupt these processes, leading to the shrinkage or stabilization of tumors and the potential for improved patient outcomes.

Welireg is an oral medication, usually taken as a capsule once daily, with or without food, at approximately the same time each day. As with any potent medication, it can cause side effects. Common ones include fatigue, anemia (low red blood cell count), hypoxia (low oxygen levels), headache, dizziness, nausea and vision changes.

While Welireg represents a significant advancement in the treatment of VHL-associated tumors, it is not considered a cure. Instead, it is an effective therapy designed to control disease progression, shrink tumors and manage symptoms, thereby improving the quality of life and potentially extending the lifespan for eligible patients. Treatment decisions, including the duration of therapy, are made in close consultation with an oncology specialist.
