Dosage:For dosage information, please check the General Information section.
Strength:1 gram Cefiderocol per vial
Indication:Fetroja exhibits a broad spectrum activity encompassing many gram negative bacteria including multi drug resistance (MDR) bacteria , Fetroja also extends its efficacy with Carbapenems, it also possesses an exceptional cell permeability which allows it to effectively penetrate the outer membrane of gram negative bacteria which acts as a barrier for other antibiotics.

FETROJA is a cephalosporin antibiotic for patients 18 years and older, used to treat infections caused by susceptible Gram-negative bacteria. It is indicated for:

  1. Complicated Urinary Tract Infections (cUTIs), including Pyelonephritis: Effective against Gram-negative bacteria such as Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, Pseudomonas aeruginosa, and Enterobacter cloacae complex.
  2. Hospital-acquired Bacterial Pneumonia (HABP) and Ventilator-associated Bacterial Pneumonia (VABP): Targets Gram-negative bacteria like Acinetobacter baumannii complex, Escherichia coli, Enterobacter cloacae complex, Klebsiella pneumoniae, Pseudomonas aeruginosa, and Serratia marcescens.

To prevent drug resistance, FETROJA should only be used for infections confirmed or highly suspected to be caused by bacteria.

STRENGTH:

FETROJA for injection is available as 1 gram of cefiderocol in lyophilized powder form, provided in single-dose vials.

DOSAGE AND ADMINISTRATION:

The recommended dosage of FETROJA for adults with a creatinine clearance (CLcr) of 60 to 119 mL/min is 2 grams every 8 hours through intravenous (IV) infusion over 3 hours.

  • For patients with CLcr less than 60 mL/min, including those on hemodialysis (HD) or continuous renal replacement therapy (CRRT), dosage adjustments are required.
  • For patients with CLcr of 120 mL/min or greater, dosage adjustments are also recommended.
  • Patients with CLcr greater than or equal to 120 mL/min, FETROJA 2 grams administered every 6 hours by IV infusion over 3 hours is recommended

The treatment duration is 7 to 14 days, depending on the patient’s clinical condition.

Estimated creatinine clearance (CLcr) Dosage  Frequency
CLcr 30 to 59 mL/min 1.5 grams Every 8 hours
CLcr 15 to 29 mL/min 1 gram Every 8 hours
CLcr less than 15 mL/min, with or without intermittent HDb 0.75 grams Every 12 hours

ADVERSE REACTIONS:

  • Hypersensitivity reactions
  • Seizzures and Central nervous system disorder
  • Hypokalemia
  • Candidiasis
  • Infusion site reaction

Storage :

Fetroja vials should be stored refrigerated at 2°C to 8°C (36°F to 46°F)

  • Increase in All-Cause Mortality in Patients with Carbapenem-Resistant Gram-Negative Bacterial Infections

Carbapenem-resistant gram-negative bacteria (CR-GNB) are associated with higher mortality rates due to limited treatment options.

  • Hypersensitivity Reactions

Hypersensitivity reactions are immune system responses to certain medications or substances, which can range from mild skin rashes to severe, life-threatening reactions like anaphylaxis.

  • Clostridioides difficile-associated Diarrhea (CDAD)

CDAD is caused by infection with the bacterium Clostridioides difficile, often following antibiotic use that disrupts normal gut flora.

  • CONTRAINDICATIONS

FETROJA is not recommended for patients who have a known history of severe allergic reactions (hypersensitivity) to cefiderocol, other beta-lactam antibiotics, or any other ingredients in FETROJA.

FAQ

Fetroja is the brand name for cefiderocol, which is an intravenous antibiotic used to treat serious bacterial infections, particularly those caused by multi-drug-resistant Gram-negative bacteria.

Fetroja is used in the treatment of

Complicated urinary tract infections (cUTIs), including pyelonephritis

Hospital-acquired bacterial pneumonia (HABP) and ventilator-associated bacterial pneumonia (VABP)

Infections caused by carbapenem-resistant Gram-negative bacteria

Fetroja is a siderophore cephalosporin antibiotic, which is a bit unique.

It attacks  the bacteria’s iron transport system to get inside, the bacteria actively take it in, thinking it’s iron which they require.

Once inside, it kills the bacteria by binding to and inhibiting penicillin-binding proteins (PBPs), which are essential for building the bacterial cell wall.

Common side effects: diarrhea, infusion site reactions, constipation, rash. Serious reactions include Liver problems – Headache, Low potassium or magnesium levels, Atrial fibrillation .Close monitoring is needed for patients with kidney disorders

Fetroja (cefiderocol) is administered intravenously (IV).Dose: 2 grams every 8 hours
(dose may be adjusted based on the creatinine clearance). Infusion time: Infused over 3 hours per dose. Duration of treatment: Generally of 7 to 14 days, depending on the severity of infection
