CMV RETINITIS 

FOSCAVIR is approved for the treatment of cytomegalovirus (CMV) retinitis in patients diagnosed with acquired immunodeficiency syndrome (AIDS). FOSCAVIR combined with ganciclovir is indicated for patients who have relapsed after receiving monotherapy with either drug. CMV retinitis is a severe viral infection that affects the retina, the light-sensitive layer at the back of the eye. If not treated, it can cause progressive vision impairment and may eventually result in blindness.

RENAL IMPAIRMENT: Renal impairment often emerges during the second week of induction therapy but can occur at any stage of FOSCAVIR treatment. Regular monitoring of kidney function is essential during both induction and maintenance therapy (refer to the PATIENT MONITORING section). In many cases, increases in serum creatinine levels subside after discontinuing FOSCAVIR or modifying the dose, though reversal is not always assured.

MINERAL AND ELECTROLYTE ABNORMALITIES: FOSCAVIR has been associated with changes in serum electrolyte levels, such as hypocalcemia, hypophosphatemia, hyperphosphatemia, hypomagnesemia, and hypokalemia. FOSCAVIR may also cause a dose-dependent reduction in ionized serum calcium, which may not be apparent in total serum calcium levels. This effect is likely due to the chelation of divalent metal ions, such as calcium, by foscarnet.

SEIZURES: Seizures linked to mineral and electrolyte imbalances have been reported during FOSCAVIR treatment. Some seizure cases resulted in death. Risk factors for seizures include pre-existing renal impairment, low total serum calcium levels, and underlying central nervous system (CNS) conditions.

PRECAUTION: It is important to administer FOSCAVIR solutions only into veins with sufficient blood flow to ensure rapid dilution and distribution, minimizing the risk of local irritation. Local irritation and ulceration of the penile epithelium have been reported in male patients, possibly due to the presence of the drug in the urine. Both male and female patients have experienced genital irritation and ulceration during FOSCAVIR treatment. Maintaining adequate hydration and paying close attention to personal hygiene may help reduce the likelihood of these events.

Contraindication:

FOSCAVIR is contraindicated in patients with significant hypersensitivity to foscarnet sodium.
