Indication:Vyloy is a monoclonal antibody used in combination with chemotherapy to treat advanced gastric and gastroesophageal junction (GEJ) adenocarcinoma. It specifically targets tumors that are HER2-negative and express the CLDN18.2 protein. Vyloy is the first FDA-approved therapy directed at this biomarker, offering a personalized treatment option.

Vyloy (Zolbetuximab) is indicated for the first-line treatment of adults with locally advanced unresectable or metastatic gastric or gastroesophageal junction (GEJ) adenocarcinoma that is human epidermal growth factor receptor 2 (HER2)-negative and expresses claudin 18.2 (CLDN18.2), as confirmed by an FDA-approved diagnostic test. It is used in combination with fluoropyrimidine- and platinum-based chemotherapy. This targeted therapy is specifically designed for tumors with CLDN18.2 positivity, a biomarker found in a subset of gastric and GEJ cancers, providing a more personalized treatment approach for eligible patients.

Vyloy for injection is available as 100mg of zolbetuximab in lyophilized powder form, provided in single dose vial.

Dosage: Vyloy is administered in combination with fluoropyrimidine- and platinum-containing chemotherapy as follows: 

  • First dose: 800 mg/m2 IV
  • Subsequent doses are: 
    • 600 mg/m2 IV every 3 weeks, or 
    • 400 mg/m2 IV every 2 weeks
  • Treatment is to be continued till disease progression or any unacceptable toxicity is seen. 

Side-effects: 

  • Allergic Reactions including anaphylaxis and infusion related reactions 
  • Severe nausea and vomiting
  • Tingling or numbness of the arms or legs
  • Fatigue 
  • Decreased appetite

Storage:

  • Store Vyloy vials refrigerated at 2ºC to 8ºC (36ºF to 46ºF). 
  • Do not Freeze
  • Vyloy poses a risk of hypersensitivity reactions, including severe anaphylaxis and potentially fatal infusion-related reactions (IRRs), necessitating careful monitoring during administration. 
  • Vyloy is also classified as an emetogenic agent, with nausea and vomiting most frequently observed during the initial treatment cycle. To mitigate these effects, patients should receive antiemetic premedication prior to each infusion and be managed with additional antiemetics or fluid therapy as required. 
  • Depending on the severity of adverse reactions, infusion may need to be interrupted or VYLOY permanently discontinued.
