Dosage:Recommended dose is 45 mg orally twice daily in combination with encorafenib
Strength:Tablets: 15mg
Indication:MEKTOVI is a kinase inhibitor indicated, in combination with encorafenib, for the treatment of patients with unresectable or metastatic melanoma with a BRAF V600E or V600K mutation, as detected by an FDA-approved test.

MEKTOVI (binimetinib) is an oral targeted therapy medication. It belongs to a class of drugs known as MEK inhibitors.MEKTOVI works by blocking a protein called MEK, which is a key component of this pathway. By inhibiting MEK, binimetinib helps to slow or stop the growth of cancer cells. When combined with a BRAF inhibitor like BRAFTOVI, it targets two different points in the same pathway, making the treatment more effective and potentially delaying resistance.

DOSAGE:

The recommended dose is 45 mg orally twice daily in combination with encorafenib

ADVERSE  EFFECTS:

  • Cardiomyopathy (Heart Problems): Reduced heart function (ejection fraction), leading to symptoms like shortness of breath, swelling in legs/ankles, feeling tired, or palpitations.
  • Ocular Toxicities (Eye Problems): Retinal detachment, retinal vein occlusion, or inflammation of the eye. Symptoms can include blurred vision, loss of vision, flashing lights, floaters, or eye pain.
  • Dermatologic Toxicities (Skin Problems): Severe skin reactions, new primary skin cancers (including squamous cell carcinoma and basal cell carcinoma).
  • Hepatotoxicity (Liver Problems): Elevated liver enzymes (ALT, AST) and bilirubin. Symptoms can include yellowing of the skin or eyes (jaundice), dark urine, or pain in the upper right abdomen.
  • Rhabdomyolysis / Creatine Phosphokinase (CPK) Elevation: Severe muscle breakdown that can lead to kidney damage. Symptoms include severe muscle pain, tenderness, weakness, or dark urine.
  • Venous Thromboembolism (Blood Clots): Blood clots in the veins, especially in the legs (DVT) or lungs (pulmonary embolism). Symptoms include pain or swelling in an arm or leg, shortness of breath, chest pain, or rapid heartbeat.

STRENGTH:

Tablets: 15 mg

SIDE EFFECTS :

  • Fatigue: Feeling unusually tired or lacking energy.
  • Nausea, Vomiting, Diarrhea: Digestive issues are frequent.
  • Abdominal Pain: Discomfort in the stomach area.
  • Skin Rash: Various types of rashes can occur.
  • Muscle Pain (Myalgia): Aches or pain in muscles.
  • Peripheral Edema: Swelling, especially in the hands, feet, or ankles.
  • Fevers (Pyrexia): Elevated body temperature, often seen with the combination therapy.
  • Headache: Common discomfort.
  • Cough: Respiratory symptom.

STORAGE :

  1.  Eye Problems (Ocular Toxicity):

MEKTOVI can cause a variety of eye problems, including Retinal Pigment Epithelial Detachment (RPED), retinal vein occlusion (RVO), and other visual disturbances such as blurred vision and decreased visual acuity.

  1. Heart Problems (Cardiomyopathy):

MEKTOVI can weaken the heart muscle, leading to cardiomyopathy or heart failure. This is often asymptomatic in the early stages but can progress.

  1. Bleeding (Hemorrhage):

MEKTOVI can increase the risk of bleeding events, which can range from minor nosebleeds to more serious internal bleeding.

  1. Skin Reactions (Dermatologic Toxicities):

While not as common with MEKTOVI alone as with other BRAF inhibitors, skin reactions, including rash, dermatitis, and photosensitivity, can occur.

  1. Lung Problems (Interstitial Lung Disease/Pneumonitis):

In rare cases, MEKTOVI can cause lung inflammation or scarring, known as interstitial lung disease or pneumonitis.

  1. Liver Problems (Hepatotoxicity):

MEKTOVI can affect liver function, potentially leading to liver damage.

  1. New Skin Cancers (Cutaneous Squamous Cell Carcinoma):

While MEKTOVI is used to treat melanoma, it can paradoxically increase the risk of developing new skin cancers, specifically cutaneous squamous cell carcinoma. This is more common when MEKTOVI is used with a BRAF inhibitor like encorafenib.

  1. Embryo-Fetal Toxicity:

MEKTOVI can harm a developing fetus.

What is MEKTOVI (binimetinib) and what does it treat?

MEKTOVI is an oral targeted therapy that inhibits the MEK protein. It’s primarily used in combination with BRAFTOVI® (encorafenib) to treat advanced melanoma with specific BRAF V600E or V600K mutations, and also for specific BRAF V600E mutated non-small cell lung cancer.

How does MEKTOVI work to fight cancer?

MEKTOVI works by blocking the MEK protein, which is part of a signaling pathway (MAPK pathway) that is often overactive in BRAF-mutated cancers, leading to uncontrolled cell growth. By inhibiting MEK, the drug helps to slow or stop cancer cell proliferation.

Is MEKTOVI taken alone or with other drugs?

MEKTOVI is almost always prescribed in combination with BRAFTOVI(encorafenib). These two drugs work together to more effectively target the cancer, often yielding better results than either drug alone.

How is MEKTOVI typically dosed and supplied?

MEKTOVI is taken orally, usually twice a day, approximately 12 hours apart, with or without food. It comes as film-coated tablets in specific bottle counts. Your doctor will determine the precise dose based on your condition and response to treatment.

What are the most common side effects of MEKTOVI?

MEKTOVI is an oral targeted therapy that inhibits the MEK protein. It’s primarily used in combination with BRAFTOVI® (encorafenib) to treat advanced melanoma with specific BRAF V600E or V600K mutations, and also for specific BRAF V600E mutated non-small cell lung cancer.

What are the serious side effects I should watch out for?

Serious side effects can include heart problems (cardiomyopathy), eye problems (ocular toxicities like retinal detachment), severe skin reactions, liver problems (hepatotoxicity), severe muscle breakdown (rhabdomyolysis), and blood clots (venous thromboembolism). It’s crucial to report any new or worsening symptoms to your doctor immediately.

What should I do if I experience side effects?

Always inform your healthcare team about any side effects you experience, even if they seem minor. They can offer advice on managing symptoms, adjust your dose if necessary, or determine if immediate medical attention is required for more serious reactions. Do not adjust your dose or stop taking MEKTOVI without consulting your doctor.

Will I need special monitoring while on MEKTOVI?

Yes, close monitoring is essential. This often includes regular blood tests (to check liver enzymes, CPK, blood cell counts), eye exams (before starting and periodically during treatment), and heart function tests (like echocardiograms) to monitor for potential side effects.
